Big or small – tell us what your child’s done to make you burst with pride

If you care for a disabled child who's done something - big or small - that's made you beam with pride, why not tell us about it and nominate them in the Proudest Moment category of our Together at 40 Awards.

The Proudest Moment category is one of seven award categories you can nominate yourself, friends, family or a local group for to help us celebrate UK families with disabled children or those with rare conditions.

The deadline for nominations is Monday 18th February and shortlisted families will be invited to a very special awards ceremony later this year.

It's really simple to nominate, just fill in this form and explain why your nominee is a deserving winner.


There are seven award categories, listed below. You can enter as many as you like.

Parent Carer Award

We want to hear about you or someone you know and the amazing things they do to make sure their disabled child and family can take part in everyday life. 

Sibling Award

Tell us about the often unseen but great things siblings do for their disabled brothers and sisters or that help make mum and dad's lives just that little bit easier.

Grandparent Award

Your opportunity to tell us about all the wonderful and everyday things grandparents do for your family.

ParentPower Award

Nominate an individual family or a group of parents who have campaigned or worked together to make life better for other families with disabled children in their community.

Proudest Moment Award

Big or small, this is your chance to tell us about the challenges your disabled child has overcome that fill you with pride and makes you smile. 

Excellence in Parent carer Participation Award

For local parent carer forums who have gone that extra mile to champion partnership work and co-production with local councils or health services. 

We Care about Rare Award

This is your opportunity to nominate a child or family - it could be your own! - with a rare condition and how they champion being rare.


A panel of independent judges will choose a shortlist for each category. The shortlisted families and groups will be invited to an exciting gala awards evening at a central London location, where the winners will be announced.
